1. Acceptance of Terms
These Terms and Conditions ("Terms") govern your use of the Relief Connect platform and services ("Services"). By accessing or using Relief Connect, you agree to be bound by these Terms. If you do not agree, do not use our Services.
2. About Relief Connect
Relief Connect is a lead routing and documentation facilitation platform that connects patients seeking durable medical equipment (DME) with licensed DME providers and prescribing healthcare professionals. Relief Connect does not provide medical advice, prescribe equipment, or directly fulfill orders. We facilitate connections and gather necessary documentation only.
3. Eligibility and Use
- You must be at least 18 years of age, or have parental or guardian consent if you are a minor
- You must provide accurate and complete information
- You consent to Relief Connect sharing your information with DME providers and prescribing practitioners for the purpose of obtaining equipment
Healthcare providers and DME companies must have separate service agreements with Relief Connect.
4. Services Provided
Relief Connect provides:
- Patient intake and eligibility verification coordination
- Routing of qualified leads to appropriate DME provider partners
- Documentation facilitation between patients, providers, and prescribers
Relief Connect does not:
- Provide medical advice or diagnosis
- Prescribe or recommend specific equipment
- Fulfill, ship, or deliver equipment
- Guarantee approval of any equipment request
- Act as a healthcare provider or medical practice
5. Your Information and Privacy
By submitting information through Relief Connect, you consent to our collection, use, and disclosure of your information as described in our Privacy Policy and HIPAA Privacy Notice, including sharing with DME providers and healthcare professionals to facilitate your equipment request.
You authorize us to verify your insurance eligibility and to contact you regarding your request.
6. No Medical Advice or Guarantees
Relief Connect does not provide medical advice. All equipment requests require approval from a licensed healthcare provider. We make no guarantees regarding:
- Insurance coverage or eligibility determinations
- Approval of equipment prescriptions
- Delivery timelines or fulfillment
- Product quality or suitability
- Treatment outcomes
Patients should consult with their healthcare providers regarding medical decisions.
7. Intellectual Property
All content, features, functionality, software, and technology of the Relief Connect platform are owned by Relief Connect and protected by intellectual property laws. Users receive a limited license to access and use the Services but do not acquire any ownership rights.
8. Privacy and Data Security
Your use of Relief Connect is governed by our Privacy Policy and HIPAA Privacy Notice. We implement industry-standard security measures as required by HIPAA to protect your personal health information.
9. Termination
We may suspend or terminate access to Services if:
- You violate these Terms
- You provide false or misleading information
- We are required to comply with legal obligations
You may terminate by:
- Discontinuing use of our Services
- Requesting deletion of your information as permitted by law
10. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, RELIEF CONNECT SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF OR INABILITY TO USE THE SERVICES, INCLUDING BUT NOT LIMITED TO DAMAGES FOR LOSS OF PROFITS, DATA, OR OTHER INTANGIBLES.
11. Indemnification
You agree to indemnify and hold harmless Relief Connect from any claims, damages, or expenses (including legal fees) arising from your violation of these Terms, applicable laws, or any fraudulent, false, or misleading information you provide.
12. Modifications to Terms
Relief Connect reserves the right to modify these Terms at any time. We will provide notice of material changes via email or platform notification. Continued use after changes constitutes acceptance of modified Terms.
13. Governing Law and Disputes
These Terms are governed by the laws of the State of Wyoming. Any disputes shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association, except that either party may seek injunctive relief in court for intellectual property or confidentiality violations.
14. Miscellaneous
- Entire Agreement: These Terms, along with our Privacy Policy and HIPAA Privacy Notice, constitute the entire agreement
- Severability: If any provision is found unenforceable, remaining provisions remain in effect
- No Waiver: Failure to enforce any right does not waive that right
